COVID 19 Safety
I want to assure you of my continued commitment to maintaining a safe and healthy workplace and that I am taking additional measures to protect you, your coworkers, and your families from the spread of COVID-19. As part of those efforts, I am implementing a new Social Distancing Policy and Disinfection procedures. Symptoms of COVID-19 include: Fever, Fatigue, Dry Cough, & Difficulty Breathing.
I do not currently have, nor have experienced the symptoms listed above within the last 14 days.
I have not been diagnosed with COVID-19 within the past 30 days.
I have not knowingly been exposed to anyone diagnosed with COVID-19 within the past 30 days.
I have not traveled outside of the country or to any city considered to be a “hot spot” for COVID-19 infections within the past 30-days.
Client understands & agrees that Shane Qureshi DBA Rocketlight Lab, LLC, employees, and subcontractors cannot be held liable for any exposure to the COVID-19 virus.
